About the Family Health Team

The North Renfrew Family Health Team  is committed to providing high-quality, team-based primary care to support the health and well-being of our community.

Our team offers comprehensive services for rostered patients, including preventative care, chronic disease management, and support for a wide range of health needs.

In addition to services for our rostered patients, the Family Health Team also offers a variety of health promotion programs and community-based services that are open to the public. Whether you’re seeking support for preventative screening, healthy eating, women’s health services, gender affirming care, or diabetes management, we’re here to help you live well and stay well.

Comprehensive primary care is provided through a collaborative team of professionals, including family physicians, nurse practitioners, registered practical nurses, social workers, dietitians, other allied health providers, and administrative team members. By working together and drawing on each team member’s expertise, we ensure patients receive timely, coordinated, and person-centred care tailored to their individual needs.

Looking for a Family Doctor?

Residents who are looking for a family doctor or nurse practitioner should register with Health Care Connect by calling 1-800-445-1822 or visiting the Health Care Connect website. Please note that the Family Health Team does not maintain a waitlist for patients seeking a primary care provider.
